Our main objective in collecting personal information from clients is to facilitate their treatment. For instance, we gather details about a client's medical history, including familial background, physical condition, functional status, and social circumstances. This information helps us assess their healthcare requirements, offer informed advice, and deliver the healthcare services they choose. Additionally, we gather baseline health and social data to track changes over time as part of ongoing healthcare services.

Registered Massage Therapists are regulated by the College of Massage Therapists of Ontario (CMTO). The CMTO may inspect our records and interview our staff as a part of their regulatory activities in the public interest. In addition, as professionals, we will report serious misconduct, incompetence or incapacity of other practitioners, whether they belong to other organizations or our own. Also, our organization believes that it should report information suggesting serious illegal behaviour to the authorities. External regulators have their own strict privacy obligations. Sometimes these reports include personal information about our clients, or other individuals, to support the concern (e.g., Canada Customs and Revenue Agency, Information and Privacy Commissioner, Human Rights Commission, etc.) have the authority to review our files and interview our staff as a part of their mandates. In these circumstances, we may consult with professionals (e.g., Lawyers, Accountants) who will investigate the matter and report back to us.

The cost of goods/services provided by the organization to clients is often paid for by third parties (e.g., motor vehicle accident insurance, private insurance). These third party payers often have the client's consent or legislative authority to direct us to collect and disclose to them certain information in order to demonstrate client entitlement to this funding.

Clients or other individuals we deal with may have questions about our goods or services after they have been received. We retain our client information for a mandatory minimum of ten years after the last contact to enable us to respond to those questions and provide these services.

In accordance with the regulations set forth by the College of Massage Therapists of Ontario, we maintain client files for ten years beyond their last treatment date. For secure disposal, paper files containing personal information are shredded, and electronic information is deleted when no longer required.
Your Rights
With only limited exceptions, you possess the right to access the personal information we hold about you. In most cases, a simple request is all that's required. We are available to assist you in identifying the records related to your information, and we will make efforts to clarify any aspects you find challenging to comprehend, such as abbreviated forms or technical terminology.

To ensure the security of your information, we may need to verify your identity if you are not known to us before granting access. Please note that, in certain cases, we may levy a fee for fulfilling these requests. If necessary, we may ask you to formalize your request in writing. In the event that we are unable to provide access, we will make every effort to notify you within 30 days and elucidate the reasons behind this decision to the best of our ability.

If you believe there is an error in the information we hold, you maintain the right to request corrections. This pertains specifically to factual data and not to any professional opinions we may have formed. Should we require verification that our records are incorrect, we may request supporting documentation.
